As a Community Support Specialist, you will provide rehabilitation, recovery, and community support services to individuals receiving Assertive Community Treatment (ACT) services. You will assist individuals in developing life skills, managing symptoms, accessing resources, coordinating care, and achieving treatment goals using person-centered and evidence-based approaches. Key Responsibilities:

· Provide community-based rehabilitation, recovery, and support services to individuals experiencing mental health and/or substance use disorders

· Deliver substance use counseling utilizing person-centered, strengths-based, motivational, and harm-reduction approaches

· Assist individuals in developing interpersonal, social, family, community, educational, employment, and independent living skills

· Support individuals in identifying strengths, overcoming barriers, and achieving recovery and resiliency goals

· Collaborate with individuals, families, treatment team members, and community partners to develop, implement, and update individualized treatment plans

· Coordinate care and maintain communication with healthcare providers, referral sources, schools, employers, courts, landlords, and natural supports

· Provide crisis intervention and connect individuals with resources needed to stabilize urgent situations

· Educate individuals and families regarding mental illness, substance use disorders, medications, recovery principles, and relapse prevention strategies

· Assist individuals in accessing housing, employment, educational opportunities, healthcare services, entitlement benefits, and other community resources

· Support wellness and recovery planning through skill development, medication education, healthy lifestyle promotion, and chronic disease management coordination

· Facilitate discharge planning, transition services, and continuing recovery supports

· Maintain timely, accurate, and regulatory-compliant documentation utilizing collaborative documentation practices

· Participate in daily ACT team meetings, treatment reviews, and multidisciplinary team collaboration

· Represent the organization professionally and perform other duties as assigned

Bachelor's degree in one of the following fields of study: Psychology, Social Work, Sociology, Education, Criminal Justice, Family Studies, Counseling, Recreational Therapy, Human Services, Human Development, Child Development, Gerontology, Behavioral Science, Rehabilitation Counseling

Associate of Applied Science in Behavioral Health Support degree from an approved institution

Work experience in a related human services field may substitute for educational requirements. *Related fields may include direct support such as case management, residential support, developmentally disabled support, vocational rehabilitation, and more. Four years of qualifying experience minimum or a combination of education and experience equaling four years required.

Qualified Addictions Professional (QAP) - CADC, CRADC, CRAADC, CCJP, RADC-P, RADC, CCDP, CCDP-D]

· Mental Health Professional as defined by Missouri 9 CSR 10-7.140 (2)(QQ)

· One year of training or supervised experience in substance use treatment preferred

· If less than one year of co-occurring disorder treatment experience, must actively acquire 24 hours of co-occurring training content and receive approved supervision

· Experience providing services to individuals with mental illness, substance use disorders, or developmental disabilities

· Knowledge of rehabilitation methods, recovery principles, community resources, behavioral health services, and case management practices

· Ability to communicate effectively verbally and in writing while maintaining professional documentation

· Ability to evaluate services and provide meaningful input regarding client care and program effectiveness

Additional Qualifications:

· Must be at least twenty-one (21) years of age to operate a company-owned vehicle

· Must be at least twenty-three (23) years of age to operate a company-owned fifteen (15) passenger van

· Valid driver's license in state of residence with an acceptable driving record

· Current automobile insurance required

· Must have reliable means of transportation in order to transport clients in a personal vehicle

· Successful completion of background screenings, including criminal, driving, abuse/neglect, and fingerprint checks

· Must attain DMH Community Support 101 Training within thirty (30) days of hire

· Must attain First Aid certification within three (3) months of employment and maintain certification upon renewal

· Must attain CPR certification within three (3) months of employment and maintain certification upon renewal

· Must attain Behavioral Management training within thirty (30) days of employment or first available class after hire

· Must attain required ongoing training hours in accordance with state regulations and agency requirements

· Must attain Medication Training upon hire and annually thereafter

· Must be willing to become licensed in additional states other than the state currently licensed in

· Must be willing and comfortable providing telehealth services

· Knowledge of program policies, procedures, and applicable regulations

Physical Requirements

· Perform sedentary work exerting up to 10 pounds of force occasionally and a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects

· Perform repetitive hand, finger, and arm movements for typing, writing, and documentation throughout the workday

· Sit for extended periods while completing documentation, meetings, telehealth services, and administrative duties

· Occasionally stand or walk for brief periods while performing essential job functions

· Maintain the physical ability to support community-based service delivery activities as needed for the position*
